Our forms are on our patient portal, not on this website. Fill them in before you come, or arrive fifteen minutes early and we will help.
What happens

Use the booking link or call. Tell us if you need a longer visit.

Fill in the patient forms ahead of time, or at the front desk.

Step 3: The visit
An eye exam, with the doctor explaining each finding.

Step 4: Before you leave
A copy of your prescription, a written estimate for anything you pay yourself and the next visit, if there is one.
